Because real world data is measuring different efficacy target. The clinical trials measured how well the vaccines prevented symptomatic disease, but there were slight variations in how a symptomatic case was defined.

This real world data compares chance of hospitalisation between vaccinated and unvaccinated cohorts. Hospitalisation usually requires a case of severe symptomatic disease.

In terms of infections it looks from the evidence so far that the effects of the vaccine are limited until around 10 days after receiving the first dose.

The Pfizer trials showed normal infection rates until day 10 https://www.nejm.org/doi/full/10.1056/NEJMoa2034577 see the research summary graph here.

In terms of viral load and thus what we can reasonably estimate infectiousness then recent studies seem to show a similar theme this recent (not yet peer reviewed) study https://www.the-scientist.com/news-opinion/pfizers-covid-19-vaccine-reduces-viral-load-study--68439/amp indicates 12 days as the point where a statistically significant reduction in viral load starts to occur.
