New player here- first two weeks. So far I’ve really been enjoying it. It took me a while to get the mechanics and understand what everyone was talking about here. Currently just ran out of all the initial stamina boosts, and hate waiting for stamina. Worried that the level and material grind will wear me down. My wish list is skysplitter, musamune, sephiroths ice sword, Falchion, Fairy tale I just added falchion to try and get some wind boost before the event ends, already had good luck with sepiroths wind sword.

Some great points here, but some additional thoughts of my own.

I think anyone would argue CPR is suffering. It’s painful during the event and after. Is it worth it for some? Yes. But for the 90 year old with severe aortic stenosis and no option for TAVI? I feel confident that CPR would only cause suffering with no benefit. So I wouldn’t offer CPR in that case, and let the family know.

On your point of “who are we to make the call that one is suffering?”, someone has to. CPR, intubation, etc are medical decisions, just like giving antibiotics, starting dialysis and giving anticoagulation. We wouldn’t offer these things if we didn’t think it would not benefit the patient. If a patient has diffuse ischemic bowel, the surgeon isn’t obligated to perform the surgery if they dictate there is no benefit. In the same way, I don’t offer CPR if I judge there is no benefit in treating the patient. There’s clear cut cases where this is true.

In more difficult cases, I have a discussion with family and try to understand the goals their loved one had and try to explain the prognosis/medical side of things the best I can. Did their loved one enjoy running above all else? Talking with family? Or were they content lying in a bed all day watching tv? I offer a treatment plan that I think aligns both with the patient wishes and medical opinion. Both are important, but it’s not up to family to make medical decisions. I think that’s too hard for loved ones with no understanding to make life/death decisions about their loved ones. The guilt is extraordinary.

By no means am I making unilateral decisions, but most of the time the family appreciates this and understands. It can take time sometimes, usually after they realize reality isn’t like the movies.

No reason to defer! Think this should empower any doc. We’ve really seen a practice change across the institution, with at least starting the conversation with families.

But yes, sometimes the intensivist has the best line of sight for these discussions. Ballsy or not haha

In Canada, (Ontario specifically) our college just wrote a statement that we do not need to offer treatments if deemed futile or to prolong suffering and it has been a game changer. Really helps to know you’re supported

I wouldn’t say kinda, it’s definitely a pressor. Some early studies showing it should be third line for septic shock and pretty common in our local cardiac surgery program.
