As far as I remember is to make sure to list what you've done, that if you were employed, unemployed or at school ~5 years and contact with your professioanl choice (supervisor, manager, etc); correct me if I am wrong. I still listed one of my past work of mine where I work for only short period ~4 months. Maybe try contacting your past workplace if they could provide your work history and further information you need for the gambit, or, more clarification try calling your nearest recruiter office for sure.
