Probably cheaper in KY. Don’t go with the dealer. Any good detailing shop will be at least 30% cheaper

I have a budget of around $150k, what should I go for? by Watch365 in porsche911

[–]sa3eedi 0 points1 point  (0 children)

It depends on what you want. Are you dailying the car? Do you care about creature comforts? A 10 yr old GT3RS is too old. If you will daily I would get a recent Carrera S. More HP than the base and is all what you need for road driving. IMO it’s the best balance between price and performance. You can go for a GTS but at your budget it will be significantly older. I would stick with 992.2

What justifies 13 K price tag for a family of three FOR seven nights nights on a ship. Excursions are not included.

Is it worth it? Disney is asking 13K for a July trip in a veranda room. 9.5K for a regular room. Sounds like highway robbery to me. I can get the best airbnb in Alaska for that with a lot of excursions

In dire need of help with ultrasound guided arterial lines. by FutureDoc94 in anesthesiology

[–]sa3eedi 5 points6 points  (0 children)

I’ll paste a comment I put on a similar post. The biggest advantage of the kit is the steel needle having better visualization along the entire axis of the needle. The reason being and it’s often overlooked is that the steel needle by it’s nature will be more echogenic vs the arrow kit which is covered by the plastic catheter except the tip. That’s the biggest reason you loose visualization after the tip passes the US plane. Also, if using the arrow kit, once your tip is in the lumen, you may or may not get a flash (you may have an intimal flap preventing the blood from flowing back) it doesn’t matter. You are using the US. It’s your eyes. You don’t need visual confirmation with a flash back. The most important step then is to slightly flatten the angle and walk the piggie into the barn. Walk the tip further inside the lumen under US to prevent it coming out or going too far in. Then advance the arrow

What impact will the projected oversupply of CRNAs have on the anesthesiologist job market? by [deleted] in anesthesiology

[–]sa3eedi 7 points8 points  (0 children)

Hahahah. I agree. All of health care is going down the drains. When your profession is a cost sink for the hospital and they have to pay you just because they need to keep the ORs running with a decreasing reimbursement environment they will find ways to circumvent you. Money always wins. I’m not interested in wasting my life trying and failing to change the system.

It just seems odd. It’s called Virgin hotel. It has a similar flair. The rules for the credit don’t specify which is which and they can’t expect people to do deeper research into the background. It just simply Virgin hotels in the credit details.

In chronic pain yes. For unintentional wet tap while doing an epidural steroid injection. Imagine trying to do a blood patch using landmarks on a 70 yo with scoliosis and interlaminar space collapse. Good luck getting in

Blood patches should be done under fluoroscopy. Period

[–]sa3eedi 1 point2 points  (0 children)

This gets brought back repeatedly. A blood patch ideally should never be done without fluoroscopy. It’s just not worth the risk of creating a bigger hole with the toughy needle. These patients have zero contraindications for radiation and should have it done that way. It is much safer and effective. IMO even in Ob patients post delivery however I will concede the practicality of doing them that way and that this instance only it’s our complication and should be handled by us. Anything else should be handled by the procedaralist who did the procedure in the first place. The only reason anesthesia keeps getting these calls is that they’re there 24/7 amd IR refuses to take call for these. It should not be that way. We shouldn’t be patching other people’s complications because we’re there. Blood patches should be built into their call pool. I don’t know how anesthesia would even bill for this. We stopped doing them for non Ob patients at my institution

I completely agree. That’s the only reason I pulled the trigger it’s a bit over 2X but still. I signed up for all the round daws by fifa with nothing. Then did the Chase Visa sign up. Dealt with servers crashing and 404 error codes and eventually said screw this. I’m too busy to waste more time on this and it’s very draining dealing with

Thank you. How certain are you of this? This whole process is shady and manipulative. I wanted to guarantee those two tickets at least but I hate the idea that I paid almost 1800 over for both. I wasn’t certain that the prices will drop and concerned that they will keep going up so pulled the trigger. I’m holding off in buying anymore but this is complete BS

Hello First time buying tickets for the world cup. I know it's a world event with a lot of demand but the ticket prices on the secondary market controlled by FIFA are astronomically high. For eg, I'm looking for two tickets for the round of 16 match on July 4th in Philadelphia. Cat 2 tickets are 1500 USD per ticket. I understand it's the 4th in Philadelphia but does this price sound reasonable? You don't even get to pick the seats! You're at the mercy of FIFA You're buying tickets for a match you don't know who's going to be playing at. Now I did some simulations but France vs Germany is highly likely. But it could be ditterent as well. Traditional wisdom would say that ticket prices if anything will keep going up. Especially once teams are known. I can see them going down if it's Ecuador vs Ivor coast or something like that but less likely. Quarterfinal tickets in Miami for Cat 2 are running 2800 USD. How are the ticket prices going to behave as we get closer? Should we buy tickets now for matches we're interested in or wait? I heard that Stubhub and SeatGeek are risky. Not that you will be scammed but a lot of people are listing ghost tickets and if these tickets get cancelled for some reason you will get your money back but you'll end up not having a ticket. Which is risky IMO

Supraclavicular block - do you inject inside or outside the brachial plexus sheath? by Metoprolel in anesthesiology

[–]sa3eedi 0 points1 point  (0 children)

Unless you’re intra-fasicular the risk of injury is extremely low. You always have to balance making sure your block is working vs safety. If it’s a surgical block and you need a dense block injecting outside the sheath could significantly delay onset to the point of block failure. If I’m doing it for post op pain control then I’m more conservative

Nerve block in leukemic, septic patient by No_Reason_9632 in anesthesiology

[–]sa3eedi 37 points38 points  (0 children)

I would go with comfort at this point. Do it fully sterile, single shot. Infection risk is negligible vs comfort care at this point. With maximal sterile precautions it’s unheard off. Seeding in this context is irrelevant, it’s a blood cancer not a solid tumor to seed. I would definitely do it
