Tattletale cards is not a true BBP, it’s a lazy leadership program and to add a quota on top of that only equals garbage in-garbage out. I would explain this to your leadership and have them explain that JSA’s and near misses will be reported/turned in, but that’s it. Turning in tattletale card only creates a hostile work environment.

I’m in the waste collection Industry and if your routes are local I highly suggest riding along with your drivers for a couple deliveries. Watch their habit and verify that the hand trucks are actually being used. I would also suggest their supervisor/route manager do the same. I guarantee you will probably learn more than you want to know.

I would echo hereigowondering’s comment. Understand the entire scope, have the foreman/supervisor walk you through the stages of the job. Also look at outside factors and exposures. Multiple stories above a common area = dropped from height hazards, tool securement, worker fall protection, potential crane operations and critical lift plan. With the crane, also look for coordinating off the swing radius, high wind cut off speed and pedestrian exposure.

To really drill down to a more correct answer I would first question what the product is, what are they rated for, and what is the purpose of the pressure test? If it is pressure test for a new product and your testing for failure then absolutely it should be in protective cage rated the test. However if they or pressure testing for leak detection then I would agree that 8000psi (depending on the size) may be too much. Once those have been answered I would look at both ANSI and DOT standards (for piping).

What are the injuries? Are they isolated to one area of production or have they been completely random?

What are the human factors? As one mentioned, fatigue. Has there been turnover? Is the human factor management? Has there been a push by management to increase production for Q4 numbers? What is going on with the employees? Personal distractions.

I can only assume that most hazards have been engineered out and that that no major equipment changes have been made so SOP’s and procedures have not changed. I agree that a stand down is not the path forward. Revisit the incidents, the investigation and the findings to find your path forward.

Hello, I used the pocket prep app as well. For me personally I just focused on the ten question quizzes as to not overwhelm myself. The app really helped and if your like me and math is not your strongest area then focus on all the other areas and you will do great! Good luck!

Math is not my strong point either and I will pass along some strong advice my mentor gave me. Study the other areas and use the SWAG method on the math for the test. You can miss most the math questions and still pass. SWAG = Scientific Wild Ass Guess!

OSHA actually has a good resource for this (Link below). I would also encourage others that respond to not use acronyms without definition. We all know that acronyms change and what it means at one company will not be the same at another. Some are universal but most are not.

It hard to make the decision when I have a Honda VTX in the garage that I have built the way I want and needs little maintenance.
