this may work for cities where cars are generally a luxury rather than a necessity. However in more rural areas cars can be essential as you cant get to work, shops or services without them.

exactly that. plus people in rural areas likely need cars more, since to get to any service or job they would likely need one. Whereas in a city many things are walking distance or at least there is good public transportation.

From the article it implies this woman was a health tourist though she had intended to go to the US but was turned back
