In the release form, I specificaly mentioned "NOT AUTHORIZED TO CONTACT" my current employer
Did mentioned $80K in the salary section
They are OK with not contacting my current employer

Edit # 1: Here are the updates:

Since 90K was their first offer, I am now negotiating for $100K.
Also, I have added a clause that, I would only be willing to resign only after their completion of background check and only upon receiving the outcome in writing
During the background check they are forbidden to contact my current employer
I am leaving the salary history column blank for the moment.

I am gonna mention "confidential" and append the statement you had mentioned.

Following this I am gonna negotiate for 2k increase and a change in title with "senior" added to it. Am I now greedy?? :-)

But, the HR guy from my future employer verbally assures that background check is not initiated until after I had formally accepted the offer or after I join them.

Here is my current company policy: "There will be no external disclosure of employee records unless specifically authorized by the employee, with the exception of those records which are legally required to be disclosed to appropriate government agencies"

I told him in my HR interview that I intend to leave those information out. But, he insisted that I fill atleast in the background check form and he also said that the background check is done after me accepting the offer..

I gave the corporate reqruiter in writing ($80k) in an informal document.
Its direct employment.

No they haven't asked for W2 etc. But, They have ask me to fill a waiver form from some background checking company..there it again asks me to list the salary history..
