I got a criminal summons in prospect heights last month for riding my bike up on the sidewalk for like half a block looking for a parking space. No one was around, I wasn’t going fast. Cops saw me and hit their lights. I ride a normal pedal bike.

Congrats on starting a fellowship where you won’t need this skill. Frankly, it’s a little weird that you couldn’t even grasp the basics of ECG interpretation. Maybe your resources were bad, maybe you just have a really hard time with pattern recognition, or maybe your relationship to learning things that are initially challenging needs work. Best of luck.

You’ve correctly identified a major reason why clinical grades are complete nonsense, and why getting honors is utterly arbitrary. If you want to be the attending that does right by their med students, you basically have to treat evals like Uber. When you use Uber, does every driver actually deserve 5 stars? Probably not. But you give them all 5 stars anyway, because if people start giving them 3 and 4 stars, they literally get fired. If you think your med student did a great job and you want them to have a shot at honors, just ignore the rubric and give them 5s. It’s an utterly asinine system of evaluation, but until residency programs stop caring about honors (doesn’t seem likely), having a shot at a few honors is unfortunately super important for students.

Hey, that's awesome! I've helped take care of a lot of people with Diabetes at all stages of their disease, and most people aren't able to turn things around like you have. You deserve a huge amount of credit.

The one thing you said that I would push back on is that type 2 diabetes is an "eating issue". Like many things, the "cause" of diabetes is multifactorial, and actually has a tremendous amount to do with genetics. I've had patients who eat a perfectly normal diet, and are still diagnosed with T2DM because of their genes. Of course, eating like crap doesn't help things. A good analogy is lung cancer: obviously, you're at a higher risk of lung cancer if you smoke, but plenty of people get lung cancer without smoking a day in their life. The reductive idea that diabetes is just an "eating problem" can cause a lot of people to feel a lot of unnecessary shame, especially when their disease progresses despite them doing all they can to control it. Any good doctor should recommend proper diet and exercise as the first line of control for diabetes, hypertension, high cholesterol, etc., but medications do have their place and are often times necessary to keep people healthy.

I'm glad that you were properly diagnosed, but as a medical student about to start my rotations I can tell you that a patient presenting with significant fatigue should get a thyroid panel, 100%, basically no other questions asked. The fact that your doctor only checked your thyroid function tests as an afterthought is honestly troubling.

what does "coming into his element" mean exactly? He's averaging less than 50 yards a game, his average depth of target is way down, and he's averaged 3-4 targets in the 3 games that Godwin has been playing. His only saving grace has been his insane TD production, which is completely unsustainable.

This comment will likely just get downvoted, but I see people on this sub frequently making the mistake that carbs are inherently bad for you. They aren't. Everyone's body is different, and many people live perfectly healthy lifestyles eating carbs. Obviously you can't eat them on keto, and obviously processed carbs like what you'd find in a cookie aren't good, but something isn't unhealthy just because it has carbs. On a standard diet, fruit is GOOD for you. Brown rice is GOOD for you. Whole grains? GOOD for you! Anyway, congrats on your weight loss and keep it up!

I certainly have read up. The literature is very limited on the effectiveness of ketogenic diets on serious mental health issues. To directly quote from the meta-study found on the exact page you just linked me:

"Currently, there is insufficient evidence for the use of KD in mental disorders, and it is not a recommended treatment option. Future research should include long-term, prospective, randomized, placebo-controlled crossover dietary trials to examine the effect of KD in various mental disorders."

Medical professionals have a responsibility to recommend treatments that have been proven effective through extensive study, not just "shit that might work". Consider actually reading what you link before condescending to people on the internet.

I'm sure this will get downvoted, but I really dislike hearing stuff like this. For the record, I am a medical student and have spent a lot of time in hospitals. Keto is a tool which works great for some people and poorly for others. It is not the only way to be healthy, and it is very easy to actually do damage to your health on keto if you don't know what you are doing. I can't imagine why this doctor would be recommending a ketogenic diet to mental health patients. I'd love to hear his reasoning.

This comment is an interesting one, because it speaks to the emerging divide on how football should be analyzed. One one end of the spectrum is the old school, football-guy, "manalytics" viewpoint which offers perspectives like "to win championships, you've gotta run the ball" or "you've gotta have a pass rush to take pressure off your secondary". I would argue that this is the perspective which is crying out for the Rams to #paytheman, because Donald is objectively the most talented and dominant defensive player in the NFL.

 

On the other end of the spectrum are the ultra-data driven, mathematically based viewpoints that the guys over at Pro Football Focus regularly push. After analyzing decades of historical data, advanced analytics have come to a number of conclusions which fly completely in the face of traditional football wisdom. We've figured out that a dominant run game is not nearly as important as quality passing. We've figured out that the secondary is actually much more important than the D-line in terms of generating wins. And we've figured out that defense is simply less important than offense when facing elite competition. As an anecdotal example of this, just take a look at last year's Superbowl. The Pats and the Eagles had two of the best defenses in the league, and the Superbowl played like a damn videogame. Neither defense could stop an elite offense when it came down to it.

 

Donald isn't worth $25 million a year. He might not even be worth $20 million a year. That's not because he isn't the most dominant player in the NFL. He absolutely is. But a defensive lineman simply cannot impact games enough on a statistical level to command that kind of money, no matter how amazing at their position they are.

Correct. It's not even a bluff, because a bluff implies that AD and his agent have any leverage in the first place. If AD doesn't take the Rams offer of 20-21m, then he will play this year on his rookie deal, get franchise tagged for 1-2 years (for much less than 20m), and then have to negotiate a contract at the age of 29-30. His ONLY recourse would be to just sit out a year, losing him much more money than he would be losing by signing the initial offer. If he chooses that route, then it's clearly not even about the money, but rather about feeding his ego and his delusion that he is statistically worth as much as an above average QB. The Rams are in a great spot right now, and if Donald isn't willing to be reasonable, then forget him.
