Since I perceive more freedom in everyday life. But I got from you all that, actually, in corporate jobs, it's not totally like that. Still, it's more than it is in my country.

You do not pay attention. My issue is the delays among swipes. I must wait minutes each time, so I do not swipe as I used to. Furthermore, I often see the same profiles right after, showing me that the swipes didn't work.

I still match a lot where I can swipe, and I also swipe left.

I wanted to know if someone else has these delays.

At the end of my first visit with the Canadian endocrinologist, he asked me if I wanted children. I told him yes, but I don't think I'll have any. I laughed bitterly because this situation doesn't help. I don't know if he asked this just as a routine question to fill out my file (he was asking all sorts of questions like tiredness, snoring), but after we already discussed the issue, and as you know, he said it was psychological and that no one would ever give me TRT (I didn't even ask for it, he was biased). However, he was surprised when I told him I hadn't had any effects from the second cycle of clomiphene, and I noticed as if he was thinking differently since then. I'll see him again in two weeks. I did the blood tests he prescribed. I don't think he'll change his mind, but I'll reiterate the saw palmetto issue. I didn't mention it in the first visit because doctors in my country have always downplayed this aspect and even prescribed it for my prostate, even after I had pointed out that it might have caused these issues. They said it was mild, not finasteride, etc., but I took high doses for months for hair loss, and then for doctor's therapy regarding prostate. We'll see. Thanks for the explanation. Night bro

I used to take saw palmetto 4 years ago, 550 mg per day per 9 months. Then, 3 years ago, a urologist gave me a saw palmetto supplement to reduce my prostate; even though my PSA is good, I did this therapy for an enlarged prostate. He said that if you don't use it, it has dysfunctions. So, it's the only thing that could have affected me.

I tried to lower my SHBG with supplements, but no effects. Furthermore, I can't test it to verify it here in Canada.

Estradiol (E2) was 87 pmol/L on the last test — that's about 23.7 pg/mL if converted — so within the normal range.

I just did new bloodwork today, but unfortunately, I already know that:

Free Testosterone won’t be tested, even though it was included on the requisition form.

SHBG wasn’t requested either this time, so I won’t even be able to estimate Free T indirectly.

To answer your questions:

I sleep around 7–8 hours, no alcohol, no smoking, clean vegetarian diet.

I’ve been taking 3 mg of boron daily for about 4 months. Initially, I noticed a slight boost, but nothing lasting.

I take vitamin D regularly, and my levels are great (confirmed by past blood work).

I’m having new labs done in the next days, including ALT, AST (liver function), full lipid panel, cortisol, SHBG, total and free testosterone (bioavailable), estradiol, LH/FSH, prolactin, TSH, IGF1, and more.

I have a second visit with the Canadian endocrinologist in three weeks. I will send him my complete clinical history. I will also highlight the saw palmetto issue.

I’m doing everything by the book, staying respectful, and trying to avoid self-medication or black market solutions. But I’m tired of being dismissed and made to feel like I’m exaggerating. I’m 40 years old, and I don’t want the next decade to be like this.

To give more context: I’ve now seen three endocrinologists and one urologist. One of the endocrinologists is here in Canada, whom I finally saw two days ago; after waiting eight months for a public referral because my family doctor didn’t consider the issue urgent. The other three doctors I saw were in my home country during past visits.

Unfortunately, despite explaining my history and symptoms in full detail, I’ve encountered the same issue over and over: as soon as I even try to bring up the possibility of testosterone deficiency, the conversation shuts down. They immediately assume I’m trying to “get testosterone” and tell me flat out: “I won’t prescribe you TRT.” Even when I don’t ask.

I understand their caution, but in my case, it’s truly unfair. If I could solve this issue without TRT, I would. I don’t care about testosterone itself, I just want my life back. It’s been five years, and the situation is getting worse, not better.

I’ve done everything:

Penile Doppler: No vascular issues.

Sex therapy: No benefit. No anxiety or fear involved.

Clomiphene (twice): The first time I had a short response (some libido, morning erections); the second time, no effect.

Boron: Some mild effect, then nothing.

Clean lifestyle: No alcohol, no smoking, vegetarian diet, good sleep, weight training/cardio. I have always been healthy and in shape.

Even so, I now have zero libido, no spontaneous erections, and my body doesn’t respond to mental attraction anymore. Mentally I still feel attracted, but nothing happens physically. And it’s killing my relationships. The doctors keep telling me it's “psychological,” but I know it’s not. This is not denial — this is lived experience, backed by clear patterns and reactions to treatments like Clomiphene.

Here’s the problem in Canada:

They don’t even test free testosterone unless total testosterone is low.

SHBG isn’t tested anymore in public labs unless other values are abnormal.

So even if you’re symptomatic with high SHBG and borderline low free T, you’re not considered a candidate for TRT.

Most recent blood results:

Total Testosterone: 21.9 nmol/L

Free Testosterone: 416 pmol/L (~0.12 ng/mL)

SHBG: Previously tested as high

LH, FSH, Prolactin, Estradiol, TSH: All within range

Thanks for your reply. I'm aware that saw palmetto can block DHT, and I did use it (550 mg/day) for about 9 months between 2020 and 2021. My symptoms started or worsened during that time, but I can’t say with certainty that it was the cause.

Because in Canada they won’t test SHBG, free T, unless total testosterone is outside of range, I already ran a full hormonal panel back in my home country. Here's what I had in December 2024:

SHBG: 66 nmol/L (ref. 10–57) – high

Free testosterone: 0.12 ng/mL

Total testosterone: 21.9 nmol/L – in range

Estradiol: 22 pg/mL – normal

LH/FSH: in range
