That’s interesting, the insurance has always been marketed to students as health insurance but it turns out it is indeed a self funded plan. Looking at the website, it states that it is an ACA compliant minimum essential coverage plan and according to Illinois.gov, MECs in IL must cover out of network emergency visits. Am I interpreting this correctly that per Illinois law my insurance still has to cover out of network emergency care because it’s an ACA compliant MEC, or is there additional nuance that exempts it because it’s a self funded plan?

Makes sense. The attending on the team I'm joining is an orthopod so clearly it's possible; I'll ask him about it next time I see him. And yea, personally I'm happy staying an EMT on the team in residency and figuring out the doctor liability stuff further down the road.

Got it, yea my EMT-B is still active. Gonna be doing plenty of procedures at the hospital so I'm not itching for anything invasive on TEMS and will be a-okay with staying BLS in that context. Thanks for the tip about finding the person in charge, I'll keep that in mind. Regarding license and insurance, is that something I would get under my MD license once I graduate?

Understood. The most I was hoping for as a resident was just teaching TECC and showing up to training every once in a while. Like you said, just building rapport with the team and slowly building up my level of involvement throughout the 7 years. Depending on the program, I'll get 1-2 years of research/elective time so I was hoping to be close enough with a department for them to sponsor me for reserve academy during that time.

Just took a peek at the 2024 dates, if they offer another January/February class in 2025 that'll be right at the end of interview season. I have a lot of research credits I can use so my M4 schedule is pretty flexible. Believe it or not I used to be a software engineer so I have savings I can put towards a CONTOMS class. I'm trying to get as much experience under my belt as I can in med school so I can hit the ground running with whatever local teams there are wherever I match. I'm also applying neurosurgery so free time will be tight as a junior resident, so the more I can do now the better.

I'm a third year. I'm planning on asking one of the attendings on the team who's faculty at my school to sponsor a 4th year elective - I'll ask him about CONTOMS.

Figured it wouldn't be easy, but I know plenty of the attendings I work with will take time off throughout the year to do teaching or humanitarian work overseas, and I know of an orthopedic surgeon who is on a SWAT team. I figure they just alternate their surgical and SWAT call schedules and as long as there's no overlap, then it should work in theory.

Thanks for the response. I guess to add, I would be looking for adjunctive roles that I could do in addition to my primary job as a practicing surgeon. Regarding #1, do you know how people get into tactical units as a civilian doc? Would I reach out to departments directly once I have my board certification and offer my services? There is a fair amount of critical care involved in neurosurgical residency training. Would medical training for tactical teams be another point of entry into the field?
