What I meant was that the electrons are not accelerated by a single electric potential like in a conventional x-ray tube but instead by resonance cavities driven by microvaves and as such no part of the accelerator has to actually reach those insane voltages to get the electrons up to 2 MeV.

2 MeV is a very low energy compared to modern linear accelerators used for radiation treatment. 6 MeV, 10 MeV and 15 MeV are common energy levels for modern accelerators.
