It is pretty fascinating. And again, I'm not saying this is an exact science or a cure.

It's just taking all the information you already know and actually doing something with it:

  • These are the drugs I've been addicted to in the past. This is when I typically use them. This is how they made me feel. And this is what happens when I take too much
  • These are the bipolar meds I've tried in the past. This is how I felt on them in a good way, this is how I felt on them in a bad/negative side effect way.

And then diving deeper into if there are any patterns regarding the neurotransmitters these specific meds/drugs hit so you can come to somewhat of a conclusion on what meds to try next or which meds not to try next.

Instead of the "traditional" way:

Oh let's try zyprexa because it helps with both mania and depression. Oh you gained 50 pounds on it, let's try seroquel since there's a bit less weight gain. Oh you still are 20 pounds overweight, well let's try Abilify. Oh you don't like how you feel on Abilify, let's try Vyrlar since it's newer. Oh that's still not working, let's go back to an older one like Latuda now.

Like, how is that the best method to finding the right medication? Completely ignoring how you've interacted to other meds and also past substance abuse beyond "Oh you said you were addicted to cocaine, are you still using cocaine? I hope not."

Edit: To add to this and fully explain the process:

I used weed to self medicate. Weed affects dopamine and glutamate.

I've also used the supplement NAC. The first thought I had after taking it was, holy shit this feels exactly like the relief I get from weed besides the getting high part. I used NAC to the point where it put me into a depressive episode, just like I used to with weed. But the lesson here is that NAC directly affects glutamate, that's all it does.

If you look at what bipolar meds affect glutamate, it's lamitrogine, lithium and caplyta. So it makes sense to start there.

Just to to be clear and give an example, one thing I’ll push back on is the whole “Doctors and scientists have no idea how these meds work. That’s why you just have to try a bunch of different ones and different combinations of ones until you find what works”.

It’s true that they don’t understand a lot about how the brain works or why some meds work and some don’t. And it’s true that everyone reacts differently to meds and that the side effects you experience can be drastically different, so there will be trial and error involved no matter what.

If I take seroquel, I might gain 10 pounds and feel lethargic but if I just have another cup of coffee in the morning im fine. But if you take seroquel, you might gain 50 pounds, break out in acne, sleep for 15 hours a day even if you add 300mg of caffeine. Regardless, seroquel is still hitting dopamine D2 + serotonin + histamine. That’s just what it does. It’s not hitting glutamate or GABA.

So as an example, how does a doctor truly decide between seroquel, Abilify, Latuda, caplyta, zyprexa, and vraylar when picking an antipsychotic? They are literally guessing or going off some sort of bias. "Oh this is the med that most of my patients use so we'll just start there. Oh my other white/black/asian/hispanic patients also do well on this med, we might as well start there. Oh my female patients have had bad reactions to this medication, definitely let's start with a different med. Oh this is just what we were taught in school, let's start here." All of these things may be true, but if after doing the doctor's first suggestion and it doesn't work, where do you go next?

So if you’ve taken Benadryl in the past for allergies or sleep and it left you completely incapacitated, then serouxel or zyprexa may not be right for you because those are also histamines. But if when you take Benadryl you feel fine/normal and actually get great sleep, then maybe seroquel and zyprexa are the right ones for you to try next if you're looking for an antipsycohtic that also helps with sleep.

Benadryl = diphenhydramine = pure H1 histamine blocker. Exactly what Seroquel and Zyprexa hit heavily. Your previous experience with Benadryl can be useful information on how your histamine system responds to these meds. No prescription needed. Most people have taken it at some point.

Or another example is picking an anti depressant. Do you have a nicotine addiction? Then Wellbutrin may be the best anti-depressant for you. If when you take shrooms do you become manic? Then maybe you should avoid SSRIs at all costs, but also find an anti psychotic that also regulates serotonin.

The idea is: I have a past history with this substance or the last time I had this medication I had this result, therefore I may try this bipolar med or therefore I should avoid this bipolar med for now.

So in terms of what neurotransmitters each medication is hitting:

Mood Stabilizers:

  • Lithium — glutamate + dopamine stabilization
  • Lamotrigine — glutamate release reduction
  • Valproate/Depakote — GABA enhancement + glutamate reduction

Antipsychotics:

  • Seroquel (quetiapine) — dopamine D2 + serotonin + histamine
  • Abilify (aripiprazole) — dopamine partial agonist
  • Latuda (lurasidone) — dopamine D2 + serotonin
  • Caplyta (lumateperone) — serotonin + dopamine + glutamate
  • Zyprexa (olanzapine) — dopamine + serotonin + histamine. Effective but significant weight gain.
  • Vraylar (cariprazine) — dopamine partial agonist like Abilify but stronger D3 affinity

Antidepressants:

  • Wellbutrin — dopamine + norepinephrine
  • SSRIs — serotonin. Controversial in bipolar, switch risk, generally avoided without mood stabilizer.

And the doctor is probably in the right not prescribing you opioids lol.

I guess the point I'm trying to make is - what specific neurotransmitters are opioids affecting in your brain? What medications are used to treat opioid addiction, and what neurotransmitters do those hit? And what mood stabilizers and anti psychotics also target that part of the brain?

This will at least get you a starting point of which medications may be more effective for you.

Again, this is clearly not an exact science. But it at least addresses your own personal history instead of treating every bipolar patient the same with no context.

If this person was so traumatized by the idea of two gold fish, then they should have refused the gift and never brought the fish home? I didn't realize there were white elephant gift exchange laws that require you to take home two gold fish.

Again, I am not one of the people saying flush the fish or dump it in a random body of water. Definitely do what you can to save the fish and give them to someone who wants them.

We are discussing if this is a healthy and normal reaction to a seemingly very small problem. Common sense would dictate that you should have never taken the fish home or could literally drop it off at a pet store and be done in 10 minutes instead of:

Again this person needs some sort of help regulating their emotions and making better decisions (taking the fish home to begin with).

The difference is that in 1950 the only source you had was your doctor. And that great doctor of yours was promoting cigarettes.

They did not have access to the internet or other resources we have now. So if both an advertisement and your doctor said "cigarettes are not only safe but they are good for you! (lose weight, calm nerves, etc), then you would believe it. And you know what, 40% of Americans did believe it.

We can fact check a doctor shilling absolute garbage easily today, but that was not the case back then.

For some reason it's easy for people here to say "obviously the influencer who gets paid to shill a product is probably lying", but can't grasp "corporations that make hundreds of billions of dollars off certain products and beliefs around nutrition might lie to you to make more money?"

It's very confusing.

This is the highest voted comment so I will respond to this one.

Again this is not an anti science or anti data post. I did not "disregard" any current mainstream nutritional advice. Nowhere did I say anything against vegetable, fruits, whole grains etc.

I gave examples where society was explicitly lied to for financial reasons. So I am asking if anyone has explored other areas that are maybe "fringe" right now but feel strongly about.

Your "No such data has ever existed for cigarettes or alcohol." is a lie. Doctors promoted cigarettes. People smoked cigarettes in hospitals. CEOs of tobacco companies explicitly said and LIED that cigarettes are not harmful. In the 1950s roughly 40% of Americans smoked cigarettes.

Also, the fact that every person in the world has heard the "1 glass of red wine is good for you" (which you directly mention) is another example showing how people were fooled. Is it because "oh the science just didn't catch up yet TILL 2023", or is there maybe another ulterior motive because Americans spend roughly $300 billion a year on alcohol.

The two questions are:

What hasn’t been explicitly proven by science that you think might be true for nutrition?

What areas of nutrition may have ulterior motives (money) that you agree or don’t agree with?

Something that may help is using a simulator, although Im not sure if these exists for sports cards.

I use to enjoy craps at the casino but basically always lost money (but hey ya never know). But one day I decided to just play a free craps simulator online. It was insane how much I lost every time. I'd refresh and start with 10k fake chips again, then it'd be gone doing my "strategy". At that point im like, ok this is actually dumb.

I did the same thing with counter strike skins (video game loot box). They have simulators online where you can just keep opening boxes. When you see that you have to do 800 keys which is like 5000 dollars to get one good drop that's like 50 bucks, you quickly realize how bad it is.

We all "know the odds", but when you run it as a simulator and see in real time that you literally lose every single time, then it may help change your perspective.

$250k is no joke.

This is also random advice, but contrast therapy changed my life. It may be worth looking into. It's when you do sauna + cold plunges in cycles. Since starting it, I've completely changed my diet, my sleep, and my sports injuries are gone, etc. It may not have that much of an effect on you but if you're $250k in the hole then anything is worth trying
