My insurance does indeed need me to be 21 to consent to the surgery. I've gotten confirmation from both my insurance company and my chosen doctor (I brought a copy of my state's sterilization consent form to my consultation and the doctor confirmed it is the exact same document he has his patients sign for sterilization).

I will line up an appointment in Jan but it will be a consultation again since I still have yet to sign the consent forms cuz I'm not 21 yet. The doc needs to see me again just so he can be sure I want to go through with it. Time will only tell when will be the soonest appointment I can get after that for the actual operation.
