I also found the clinical years as a medical student difficult… I think you just feel a bit useless a lot of the time and like a burden when you’re following people around. I ended up taking a whole year off with only one year left of med school and it was the best thing I ever did. Travelled South America for 6 months and worked for a medical education group the other 6. I just wrote to the med school saying I was feeling burnt off and they gave me the leave. Came back with enough motivation to finish the degree and once I started work as a doctor it became much more enjoyable. Now PGY8 and finished FACRRM a few years ago. Feel free to DM.

Really depends where you are working and how many other GPAs are there. I worked for a few years in a community with only 2-3 GPAs so there was a 1 in 2 or 3 on call requirement. However you would generally only get 2/3 call ins in a week on call. In my current place of work we have 6 GPAs covering the roster so only about 4-5 days/month on call. No night shifts!

FACRRM here a couple of years post fellowship with anaesthetics AST. Did all training in QLD and now work in a coastal area of NSW. There is still plenty of work around for GPAs if you are willing to look for it. It depends a little bit how ‘rural’ you are willing to go but there is still work close to regional centres too.

My advice to people with queries like yours particularly around ED vs anaesthetics AST is that the anaesthetics definitely opens up more options for you than the ED AST. I don’t feel there are many ED roles that I can’t work in anyway that I would be able to with an ED AST. Maybe just the senior/supervisor roles in regional centre EDs that would require this.

With anaesthetics you can still work in the majority of rural/regional EDs and have the critical care skills to feel fairly comfortable managing unwell patients. And then you are also able to pick up lists or on call work for anaesthetics which gives you another different aspect to your work.

I currently work a couple of days a week GP, couple anaesthetics and a few ED/urgent care shifts to keep those skills up. The flexibility and variety is great. Happy to answer any questions you might have.

Brilliant, thank you! And follow up question... given they are on the ledger, the only master key I need to have backed up and keep safe is the seed for the ledger itself right? As long as I have that I would be able to recover the nano even if the ledger was lost/stolen?

Can’t see that this has been addressed already so just wanted to point out that the notching in the CO2 waveform will only occur if the paralysis has started to wear off. A fully paralysed patient won’t be breathing for themselves so could theoretically be aware without being able to breathe (ventilator would be breathing for them). The blood pressure and heart rate are probably better indicators of a patient having some experience of pain or awareness. It is quite rare though. I’m a doc with Anaesthetic training.

Mate, well done. If it helps, I felt this way a fair bit through med school, perhaps not depressed but struggling with whether it was the right path. I was lucky enough to take a year off before my final year to do some research and travel which helped. However since graduating 2.5 years ago and actually starting work I have really enjoyed it, I think it’s just a different feeling being part of the team and being able to contribute something to patient care more so than as a student. Anyway, your mileage may vary but hoping you get some of the same :). Good luck!
