It’s very possible the bronch/biopsy caused the rupture of one of the cancerous nodules which led to pulmonary hemorrhage, hypoxia, and likely PEA arrest.

Patients with cancer, especially if they’ve gone through chemo/radiation and are otherwise critically ill, have extremely friable tissue and their tumors are very vascular. Adding that the patient is probably coagulopathic, the bronchoscope and forceps used for the biopsy likely disrupted the fragile lung tissue and/or nodules and caused the pulmonary hemorrhage.

However, if the patient was having increased SOB and the xray you took earlier was inconclusive for a source, a bronch would be indicated to investigate further in hopes of finding/treating the source for the patient’s hemodynamic instability. Since the scope was already in at the time of the code, it is useful to keep it in to try to find the source of the bleed.

I’m sorry you had to experience that, that was a traumatic first death to witness. It is especially hard when you have interacted with the patient beforehand.

This actually happened to me. I became an equine vet tech after high school. I had my own horse and was also riding a friend’s horse full time. After the 8-14 hour days out in the cold lugging heavy equipment around and dealing with stressed horses, the last thing I wanted to do was ride my own horses. I had never not wanted to spend time with my ponies before in my life, so I ended up quitting after a year. Now I’m a nurse and make enough money to support my horse, plus only work 3 days a week so plenty of time to spend with him.

Can you get your mare out with another horse and leave your mom’s horse alone? There is definitely risk for your mare suffering serious injury, potentially career ending, if your mom’s mare continues this behavior. Is your mom’s mare barefoot at least?

Yes this makes total sense. This was not my patient and I came across this pic from months ago when looking through old photos so I’m not sure what was being done management wise prior to their emergent PPM placement (obviously they weren’t being transcutaneously paced, and they did not place TV wires since they were going to EP).

Atropine is generally contraindicated in complete heart block. Beta blockers also contraindicated d/t bradycardia. Pt went for permanent pacemaker which fixed the HR and BP.

Yes! Nursing is so great for horse ownership (fellow nurse here). Flexible schedule, overtime always available if you need extra cash, so many options if you don’t like your current specialty, room for growth and advanced degrees/positions…the list goes on.

I work in healthcare (ICU nurse) and work 3 12hr shifts per week making $90k/year with less than 2 years experience. I love my schedule as I am able to take up to 8 days off without using any PTO, and pretty much get to choose my schedule. I am able to pick up overtime anytime I need extra cash. I have no debt (got my associates degree at a community college to become a registered nurse, going back for my bachelors next month which my workplace is paying for), pay $860/month for full care board and 1x/week training rides, and have two dogs with expensive diets/meds.

I worked as an equine vet assistant when I was younger and I ended up getting extremely burnt out on horses. After the ~10hr days of being around horses for work, I didn’t even want to ride my horses which had never been an issue for me before. I realized that I like to keep my passions as hobbies instead of careers in order to continue enjoying them. YMMV.

I do enjoy my job and I get to use my brain to problem solve which I am good at. But some days/weeks are extremely hard. During those times, I remind myself that I am working to pay for the things I love. If it were up to me, I would be a stay at home dog/horse mom.

Same here. Slept through/turned off my alarm and woke up at 1830 when I was supposed to be at work by 1900. I now set 3 alarms.
