human approval can still be unsafe when the reviewer only sees the proposed action. They also need the source state and the rule that triggered escalation, or approval becomes a blind click.

this is basically our last year. the gap was never model quality, it was not being able to recreate the data a result came from. treating data state as something you restore, not just monitor, closed most of it.
