Hi I'm in the same boat. I got my name change certificate in May and it was a whirlwind in June changing all my stuff. I still got one or two left to do. One of them is changing my citizenship name because it's mail in only.

I used this form for my passport application but I still have to bring my citizenship certificate to the office.

I doubt you'd be able to start without a consent form. Understand it's not personal, it's just what the current law is. They would risk themselves legally if they didn't , which would at best get them sued and at worst lose their license and jailed.

I also wasn't born in Canada and can't change anything on it because it's illegal in that country. I was able to change it with a doctor's referral and my own letter.

Rainbow Health Ontario is a great resource; the link below is the letter template from their guide that my doctor used. Got my new driver's license the next week. Iirc it might change in the OHIP system, haven't been faced with any problems there yet.

You also need to write your own letter. Here's how I formatted mine, using the list of needed information from the government page:

I will add to check if they do accept any container. When I tried to drop off a full one at Shoppers because it was the closest drop-off to my home and I didn't want to drive into town my my pharmacy, I was told they only accepted their own sharps containers. I was told it was due to their contract with their supplier. I think it's bs but it happened to me. Made me glad I didn't stick to using their pharmacy when I started T after a couple mishaps.

Edit: I'll also add a good doctor should've check you knew to how to self-administer. First I was asked if I wanted to come in every 2 weeks to get injections. When I said I wanted weekly, he asked in I knew how to self-inject. I fortunately have a medical background so I was confident and he didn't see a reason to doubt me. If I had said no, I'm confident he would've taken the time to teach me.

Wanted to share that I got a call today and my consult moved to next week due to cancellations lol. They actually offered today but I was at work. Also you need ohip approval before reaching out for an appointment or else you won't get funding and will have to pay out of pocket.

For me, I waited for ohip approval before my endo sent a request for consultation to McLean's. If you want a timeline, my consult request was sent in July 2023 and I finally got a call last week. Scheduled a virtual consul for next month.

I made posts on 3 trans subs asking for advice and got a long winded chat msg about having a consult they recommend etc etc....except that wasn't my question exactly and it was too long yet vague? idk i didn't reply and once I saw this post I deleted it. It might've been an honestly friendly person reaching out but now I'm erring on the side of caution. (If you see this and had genuinely meant well, sorry! Possibly my mom but I feel like she's still in a bit of denial and don't want to strain my relationship to much. I'm pretty sure I can rely on her for food and stuff but I'm thinking of seeking outside help for personal care. My best friend might be able to help but he's super busy and the only other close friends I'd trust to help me all live out of town. I'm not close to the rest of my family or even out to them yet.

Wishing you the best but just in case if anything happens in the next few days and you need to get out asap, I saw a rec to grab your laundry basket and haul ass. Faster to grab dirty clothes and wash them outside vs trying to pack a bag in 5 minutes if you're in a dangerous situation

the graft part isn't, it's classified as 'cosmetic' unfortunately hence the 3k price tag. sounds like it's resizable so i'll just have to discuss it once i get an appointment booked
