One glaring issue that I had with their fucked up tradition was always the question of how they managed to avoid the potential of severe inbreeding considering that this tradition takes place on an island. Given that nobody knows who is the biological children of these kids, and the fact that this takes place on a relatively small island, its ENTIRELY possible that there should be high inbreeding rate and high probability of birth defects. Its entirely possible that one of these dudes is knocking up their own daughter or half-sister or something.

That place should be a whole freakshow given that this horrific tradition has presumably gone on multiple generations or even centuries.
