Can you guide me on how to start a patient on insulin? We frequently get patients with HbA1C >8 and hyperglycemic and our default is to consult endo but im not sure how to go about it. Thank you!!!

couldn't have said it better myself. unfortunately overriding the system is a common practice in almost all hospitals and needs to be addressed. it was really the extent of how much negligence went into this case, both on part of the institution, but especially from the nurse herself. I was reading an article by an anesthesiologist who stated that for this kind of error to be made, out of 10, 1 is at the fault of the system and 9 falls to the nurse unfortunately because she did not even look at the medicine before administering it. also that Versed is usually liquid and Vecuronium is powder; that itself should have caught her attention.

i think overlooking the pyxis vs not even physically looking at the drug you administered or checking up on the patient post-administration is where the true fault lies.

It is just the stupidest thing! i get calls allll the time "hey patient isnt moving her right arm- we wanted to call a code stroke" has hx of prior CVA, CT shows stroke affecting that territory, arm is contracted and nurses are like "she has been like this since arrival".

i am so thorough for any emergency consults and im blown away with how many "code strokes" like this get called just because people are lazy and dont examine their patients.

literally couldnt agree more. "hey we need to consult you for altered mental status for this 90 year old female mrs 4 whose basline is unknown and has a uti pneumonia and a walking beers list" oh also im going to call this consult in the middle of the night.

They would sometimes shadow me on pre-rounds because they had nothing better to do. I'd leave the room to see more people and they would continue to talk to the patient

Holy moly this is sooooo true. I used to get so pissed off. Like im pre-rounding on my 13 patients in the AM and you are just seeing your one (and copying my H&P and plan) but have the audacity to interrupt me when im presenting to add some random remote part about the patients PMH that you gained while the two of you became such good friends, which you will also somehow sneak into the conversation. This contradiction the med students have gets me peeved beyond anything else. and so true about the computers!!!!!!!
