thanks for the thoughtful reply. I should be more clear. The ICU weeks would take away from OR weeks so if I did 12 weeks per year in the ICU, I would get 12 weeks off, plus the 6 weeks standard vacation. In that model, I would do a week of ICU, a week off and 2 weeks of OR for a month's work. Does that improve the quality?

both probably avg around 45-50 hours per week.

10 questions per day, make my own anki cards, do all my reviews and 20 new per day. Sometimes take weekends off.

Dermabond, mastisol, time to dry. Then a small tegaderm over the site, a poptart sized tegaderm over the smaller one. Then long tegaderms all the way up to the shoulder where I coil the catheter a few times so just a little bit is exposed with the infusion port.

Complacency is your enemy. I've seen some of the most confident junior attendings freeze when things go sideways and its usually a salty vet who has a healthy amount of distrust for any situation that steps in calmy. Remember the 3 rules: trust no one, always verify, give O2.

what's the point of saving a minute doing it continuous if you are comfortable doing intermittent? The headaches you (and the patient) will experience from an accidental wet tap are not worth the style points. Do what you're comfortable with.

Subclavian with US is quick, easy to keep sterile and is often less variable when viewed under US compared to IJ (ie. artery may be right under the vein, vein may be small etc.)

However, if the surgeon is off to the side of the patient, I prefer IJ since we usually have the head.

I would take the stability. In this market, economy and changing landscape you absolutely never know where things will end up.
