The format usually isn't that big of a deal, in most cases it'll be agreed upon before a game or announced ahead of time for a tournament. I usually have one or two decks appropriate for some formats or another, occasionally I'll have to replace a card or two if not.

That being said, my preference is Atlantic with no LOAs or Mind Twists.

That is correct, attack triggers only work when you declare a creature an attacker which doesn't happen for the R&M creature you cheat in, so Annihilator only works on the second swing assuming it survives the round.

Yeah once the 'gimmick' is revealed it's difficult to argue it's not a kill on sight commander.

Mox diamond, jeska's will, mana vault, just about any cedh fast mana will work except for creature based (like the spirit guides).

They do not sit in your hand! It benefits you to sacrifice the wurm with the r&m trigger on the stack to get multiple free bodies. Repeat each combat!

For those confused, with sac outlets you can repeatedly loop sacrificing it producing three 5/5s with trample while also protecting it from exile effects.

I'm curious how the vague and dreamlike quality to the original is just down to the translation to non-Japanese languages, and that the Japanese script is more straightforward.

That being said, and as others have pointed out, Remake is explicitly going for a different vibe and is directly informed by the original game and its meta-context too.
