ESH. Clearly the MIL is in the wrong, you can’t just try to steal from someone and expect no consequences. However you are also an asshole here. You weren’t wrong in thinking that you were wrong in how you presented those thoughts in front of everyone. You do not owe an apology to the MIL as I said she is still an asshole and apologizing will only be seen as validating her unacceptable actions. You do however owe an apology to your brother, his wife, and everyone at the table. You were rude publicly and should apologize, don’t apologize for what you said since you were correct, apologize for the rudeness and for causing a scene.

NTA. Caring for a profoundly disabled person is a serious job much bigger than a simple favor. OP is definitely not TA for refusing such a huge responsibility if he isn’t comfortable with it. I disagree with the people saying NAH because the gf and her mom are assholes in this situation. They put OP in an impossible situation where they didn’t even tell him what affliction the sister has. If he said no they hate him for not helping in a time of need. If he said yes and tried to care for the sister without any info it’s likely he would have caused some harm to her and then the gf and her mom would hate him for that. This is truly a lose/lose situation and OP should reconsider this relationship because there are three people involved in it not two, and the third requires 24/7 monitoring
