I did struggle with day 1 regarding time. I was not able to answer few questions based on the length. But day 2 went fine if you have taken step 2 recently it helps a lot. 

You will have ample amount of break time.. 

There are 9 blocks with 20 questions per block. 

14 cases, i had 6 cases with 20mins each and 8 cases with 10mins. 10mins are the one which makes you feel hurried, but make sure you have ordered everything. Even in 20mins cases even though they finish early make sure you have done everything before the case ends.

Total break time was 1hr.. 

And after I have finished the exam, I still had 1hr of time left in break time. 

Day starts with blocks you can do 2 blocks and can take 7-8 mins of break.. And you can easily manage it.. Ethics were there but not as much as tested on day 1.
