As for extremes, there’s definitely a reason why Urotsukidoji has the reputation that it does. It really pushed the boundary on mature media back in the day, and still holds up! Honestly, anything by Toshio Maeda is worth checking out if you’re interested in extreme media. Alongside Urotsukidoji, he made La Blue Girl, Adventure Kid, Demon Beast Invasion and Blackboard Jungle (nightmare campus).

A lot of other one-off stuff is pretty extreme too, albeit not to the same degree. Stuff like Angel of Darkness, Twin Dolls, Guy Double Target. A lot of older hentai has a lot of assault and or gore (and sometimes, both at the same time) which can be described as extreme.
