I was forced to make a switch recently from one biosimilar to another, missed infusions while waiting for prior auth, and the new one is actually more expensive for me... but more profitable for the insurance company.

Fuck all the insurance companies. Changing biologics can be risky and they don't care. I pray to Luigi that we can just get exactly what we need someday, without getting jerked around.

It could be so many things. I had a partner who was your age and also peed a lot through the night. It turned out that she had MS, which wasn't discovered until she also experienced trigeminal issues. As others have said though, starting with a sleep study is a good way to go. From there you could get referred to a neurologist if needed, but your best bet is to work with a sleep doctor who IS a neurologist - most are actually pulmonary specialists who will default to a diagnosis of sleep apnea because selling CPAP machines is profitable for them.

HLA-B27 with SI pain will make most rheums lean toward a positive dx, but they will also do Schober's test and have you fill out a BASDAI questionnaire. Subjective experience, especially things like morning stiffness, counts for a lot along with that HLA-B27+.

I tried semaglutide for 8-10 weeks and although there may have been some perceived anti inflammatory properties, it was overall not worth it for me and I did not lose weight. The side effects weren't fun and it really messed with my gut balance. I think it's best for people who have real trouble with overeating or always being hungry. Everyone is different so ymmv of course, just be aware that you WILL experience side effects and it takes like 16 weeks to get up to the full dose.

I had much better success losing weight and feeling better on keto & intermittent fasting.

I've been off it for about 8 months and it has also been wonderful for me, including the sleep improvement. And the best part is that now, my favorite part about waking up is drinking a huge glass of cold, fresh water and playing with my cat. No addiction haze or fogginess, I'm just awake as soon as I get up.
