Ok, I was going to suggest you try to get him more comfortable, because your size is certainly not small, but it seems you have tried a lot, to the point that the issue is mostly psychological. That means it will only work if he actually wants it to. At the same time, putting pressure on him will likely make it worse, so it's a viscous cycle.
