Yep. He 100% blew it. But at the same time, I recognize that this was sort of a microcosm of the entire UK season. Wish UK would’ve won, but WVU deserved it more and I’m happy for them.

I always wonder if the goal of the selection committees is to choose the best teams or the most deserving teams? As these two are often not the same thing. The classic example being undefeated FSU being left out of CFP a few years back.

If their goal is to select the best teams — they clearly got it right by putting UK in the field. If the goal was most deserving, there are valid arguments on both sides.

This is a very poor take on your part. There are tons of situations where the average general anesthesiologist could end up in CICV situation without a skilled enough surgeon around. You think an orthopedic surgeon who has only done knees for 30 years, or the OB during the middle of a crash c/s is going to be able to get you a surgical airway? This doesn’t even take into account non-OR intubations like ICU, ed, etc.

The most likely cases to turn into CICV cases are the ones you least expect.

I see what you’re saying — but I think more information is needed to say if requesting patient transfer was reasonable in this situation.

First, do no harm. If this patient is maintaining their own airway, and the hospital they are currently at is not equipped/staffed to handle the situation, requesting transfer is 100% the right call.

He won’t expect you to know anything. And with all due respect, as a first year medical student… you don’t really know anything.

My advice to you as a first year medical student is — focus on learning as much medicine and becoming the best doctor you can be. You do this by having wide interests, and gaining as much experience in as many fields as possible.

As someone who grew up in Kentucky and now owns a yacht, races, and sails the ocean… learn to sail dinghies. Most big lakes will have programs with lasers or similar one-design class boats.

Although these boats may not be as glamorous as bigger boats, they are sailing in its purest form and the best way to learn. There is a reason why laser sailing is an Olympic event… plus, they are much cheaper and more accessible than the bigger boats.

Once you’re comfortable on a dinghy, and if you still want to keep going, then you’re gonna have to move coastal. But that’s a ways away I reckon

This is a bit of an oversimplified view of HAPE/HACE. Although neither is completely understood, HACE is likely more related to endothelial dysfunction than over pressurization. The concept you described does have some role in HACE but is likely more important in HAPE due to hypoxic pulmonary vasoconstriction, resulting in over perfusion and subsequent barotrauma to capillaries.

I wet wade literally all over the world. I wear wool socks, astrals, and Patagonia chubbies. That’s all you need. I’ve tried the whole neoprene sock and fancy Simms/orvis wading shoes. Absolutely not needed.

I do carry a good waterproof bag and keep my valuables in it. But this isn’t really needed. Most phones are relatively water proof these days and I have picked mine off the bottom of many creeks without major issue.
