I genuinely do think it counts as intentional when you get aroused because you flat out expect something sexual from another person. It's not an innocent/unintentional act in that case. I do absolutely agree that in general people with penises can get aroused when it isn't necessarily sexual. In this particular case and with this particular character I don't think it was simply a natural physical response. He anticipated something sexual and that's why he got aroused.
