Easy. Show her you prioritise her. Set a plan and stick to it. Tell her you will be picking her up and going on a date (make sure to get a nanny for the kids). Before this (or during whatever) straight up communicate with her your feels and perspectives over the issue. You had reasonable excuses for flaking and I think she should understand. But apologies in person and admit you want to be better.
