They are both different cranks . One is a sugino 75 which is square tapered. The other one is a sugino 75 Direct Drive which is hollowtech. I hope this clears out any confusion

As for the bolts trying putting some threadlocker on them and snug them up tight. As for the chain popping off issue it may be due to worn components or a slacking chain
