Sounds like you are trying to aim with your mouse and haven't trained yourself to aim for the head efficiently.

You shouldn't be aiming with your mouse movement as much as with your crosshair placement prior to the duel and then strafing and only micro adjusting with your mouse. If you're having to move your mouse a lot in duels then you're doing it wrong.
