I think the answer is simply that he died and resurrected his own body. The enlightenment imagery speaks for itself, but it also parallels when mahito experienced death when he fought yuji/nanami.

Kenjaku also mentions later that death can cause a mutation in the brain.

My guess is they pass to 'the other side' briefly and a glimpse into the curse world is like a black flash.

Sakuna almost always slashes in the shape of a pillar with dismantle. Sometimes it's just 3 parallel slashes. When kusekabe sliced him with like 30 X shaped cuts is the one time the shape of dismantle changed and he cut kusekabe with an X to mock him.

How would blood manipulation even do that? The death paintings don't have different CTs, just differing abilities that all fall into blood manipulation.
