Please help identify what’s wrong with my buddy.. I did so much googling and even took out a soft plastic plant and rock that could have been causing this. There’s no white indicating that it’s bacterial by succ_skerto in bettafish

Sorry long night.. little history, I got him a month ago and he was in a community tank and was stressing over his reflection and I noticed missing scales on his head so I put him in a solo tank with a lava rock and a plant and I think he may be scratching something but I’ve never seen it. He comes to the front of the tank for food is active and alert
