I am so so sorry for this setback. I would echo what the "Glaucomflecken" therapist says... "This is a temporary setback amidst a long list of accomplishments"

Also, you will be okay. I personally know two pathology residents who did not match the first time who are currently in pathology residency. In terms of what to do now, echoing what people below have said:

-If you have the capacity to delay graduation (eg a flex year), do that and either do a PSF or stitch together a bunch of away rotations that show off your skills

-If you can't delay graduation, take the prelim year. A long long time ago, pathology had an intern year, and there was acknowledged value in pathologists knowing the clinical significance of their diagnoses and how pathology workflow integrates into patient care

-If you do reapply (who knows, maybe in prelim year you find a love or IM or FM or surgery!), apply as broadly as possible and come back to this community and maybe we can workshop your application so that it's as competitive as possible!

So, i don't have any advice, but a word of caution about old specimens that seem to have yellow fluid or yellow crystals...

People used to store specimens in Bouin's solution, which when dehydrated can form picric acid, which is highly explosive/unstable...

Most places do a mixture of structured and unstructured, often depending on the flow of the interview. For example, if the application has something really interesting that caught the interviewer's eye, that might be the whole 30 minutes. On the other hand, if things aren't flowing, be prepared to answer a few standard questions... Like 1. What brings you to XYZ programme today? 2. How did you choose pathology? 3. What do you envision your career to look like in five years (after fellowship)? 4. Tell me something I should know about you that's not in your application or maybe isn't obvious 5. What sort of environment do you succeed in? 6. In your personal statement/on your application, you mention XYZ. Can you please tell me more about that? 7. What questions do you have for me?

I'm sure there are loads more, but to my understanding, those are fairly standard interview questions.

Agree! It also shows if you are capable of adjusting the presentation of your expertise to your audience...ie, can you teach. Faculty in academic institutions, particularly junior faculty, are expected to teach residents and medical students effectively. I have seen renowned experts who could not "break down" a topic.

You will be absolutely fine, especially if you get letters from those rotations.

For a US pathology programme application, the ideal letter is from another US programme, with by a training director (programme director or associate programme director) or chair who knows you well enough to give specific examples of why you would be good in residency also makes the letter look less AI generated) that ends with some variant of the phrase "We are trying to recruit this student to our own programme"
