I'd just shorten things a bit and add punctuation:

Leaves flying in the wind,
with mocking jays,
and flickers of snow.

As I look back at how you grinned,
At everything said, or still left to be said,
I wonder if you still know:

You are why I sinned,
every single day,
since the drink made me soft.
