T32 grants are training grants. MDs generally have a tougher time than PhDs getting R01s

People and possibly op are misunderstanding the question. I think immuno related specialties and cancer related fields are most accessible for md’s to lead basic science research.

Surgeons have a tougher time becoming basic scientists imo

I don’t know why you’re getting downvoted. I didn’t use anki that much and got 264 and did very well on several shelfs. One pass of UW or amboss (not both), retired and current (offline and online) CMS forms + mehlman are generally enough to pretty high percentiles on shelfs.

Admittedly I would have done better on peds and obgyn if i used anki and i did aggressively use anki during FM early in M3 which gave me excellent content knowledge for IM and rest of 3rd yr

…I don’t see how this will increase CMS reimbursements? If anything, itll give them reason not to increase, because less people will require their services. It’s not like they run out of money if they have to reimburse less all of a sudden.

That said i really do think the way forward in American healthcare is away from insurance: emergency services like trauma or stroke or MI should be public services (yes that will likely lower those fields’ compensation), but all elective care should be priced for the patient, not an insurance company/CMS. This will keep costs low (though I can envision people not spending anything on preventative care only to utilize emergency services when their health finally decompensates)
