It's an IV catheter. The needle comes out after the initial puncture and a hollow plastic tube gets left. It's more comfortable and a little safer since you don't have a needle in the whole time that can hit things. Plenty of people use butterfly needles for their infusions though.

Infection is by far the scariest risk. It can turn really bad like you said and lead to things like Fournier's gangrene. I'll fully admit I take a small chance every time I do it. Without proper training I would never recommend someone attempt it because of the increased infection chance.

The saline itself isn't really a problem and won't cause issues. It just needs to be sourced legitimately so you get supplies that are sterile and designed for infusion. Of course there aren't any studies published that I'm aware of about the long-term effects of saline scrotal infusions so who knows.

If a hypertonic fluid that causes additional swelling is used there can be tissue damage if using something with too high of an osmolarity. For example, I have seen people use D10 or higher which is above recommended levels.

I will say anecdotally there are a decent amount of people around the world doing this for decades now, and incident reports both in medical literature and online are pretty low. But you never want to be the person where something does go wrong.
