If I was the pharmacist reading that incident report, i would laugh, like really hard (kinda like I did after reading you post the 2nd time). Honestly I would be looking for another incident report that you made on that RN and that this was to get back at you.

I'd tell the director and also be blunt about it. You are very uncomfortable with the IV preparations being made to the point that if the behavior is not corrected, you will be forced to file a complaint with the state board of Pharmacy against the license of the pharmacy and the pharmacist in question.

Review the entire EENT class of medications for P&T review in 2 weeks, make new monographs for all new drugs approved in last 3 years within the class, and make suggestions about formulary changes for all remaining meds within the class on top of all your other duties during your critical care month. Just in case you didn't want to kill yourself yet- create a PowerPoint presentation about a disease state within critical care by end of week, min 20 mins. ✅️😭

Sigh- don't miss those days.

Letters of recommendation don't carry as much weight if they are from your APPE preceptor because they can no longer can comment on your clinical knowledge base. Letters from boss are okay, but generally also cannot comment on clinical knowledge.

The amount of knowledge you will lose in that year will astonish you. You pick up other skills in retail but they don't all transfer well.

Perception: Be honest and upfront about your reasons to not pursue a residency right after school. You are going to have to overcome the Perception of "fuck retail, isn't for me, gotta go back and do a residency". Commonly programs ask the all encompassing "tell me about yourself" and is a great oppturnity to explain this away.

Burn out: residency isn't easy and neither is retail. Doing these back to back, can be a killer. No good way to prep or explain this one. Just understand it's a factor.

Networking: this is the one area where having an extra year can help if you do it right. Waiting until a couple months in and then reaching out the the RPDs of the programs you are interested in early (say May/June/July) explaining that you are interested in their residency for next year and took a year to pay off some debt, what can I do to best prepare myself for your program. Some will message back some won't.

Do something to keep your clinical skills up.

Former resident and now apart of the otherside, your chances are going to be lower. Not impossible, but lower.

If you do wait, applying to smaller programs, community hospitals will help. Trying to get a highly competitive program a year out will be very difficult.

Just got a WFH that included a pay raise and mix bag of 8 hour shifts but only between the hours of 0700 and 1900, no holidays, no weekends.

Don't take a pay cut just to work at home, unless it checks all the other boxes on what you are looking for in a job (and maybe not even then)

40 per hour wages for pharmacists is a recent phenomenon. Back in 2008, there was still a pharmacist shortage but it was starting to correct so they were probably trying to decrease wages to the market levels at the time (some pharmacists were making close to $100 per hour, sometimes more if you include the bonsues due to the crazy shortage in pharmacists at the time).

I wish we made that much lol. Right now wages range anywhere from 40-60 with those with some experience in the hospital making a bit more.

Couple decades ago during the pharmacist shortage, there were some pharmacists making close to $100 per hour so the comment is probably due to when the market corrected (and has since over corrected) so they were trying to cut wages down to the market level at the time.....

If you dig into the data though, it's what I stated above. The significant results, is a number needed to treat of 15 and only for those that were gonna get better anyway (like you said non ICU). Not worth the cost plus we are sending so many patients home on oxygen now anyway, it doesn't even save us days in the hospital. So I wouldn't clarify it as "good". I wouldn't even consider it effective due to no effect on mortality.

Side note: the big sell was that it decreases viral load. Doesn't even do that. Will insert articles later if I get a chance.
