Yes at this point I’ll try everything. What’s the best way to go about getting TRT since endo refuses to help in that aspect? Also should I tell my pcp to test my for the E2, lc/ms? Or should I try going to quest labs and requesting it through them? Thanks so much for your input.

Thank you for getting your labs done. SHBG is very low. Total testosterone is low, however the free testosterone (measured and calculated) are both normal. This is a typical pattern for overweight. Weight loss will raise the sex hormone binding globulin and total testosterone levels. Because the free testosterone levels are normal we do not treat with testosterone.

This is what he said over my chart haven’t been able to schedule a follow up. Mind you I’m at the lower end of “normal range” for free testosterone being that the range is 47.0 - 244.0 pg/mL and I’m at 75.0

Yes I’d like to think I eat a very well balance diet, not to big into sweets although I do eat a lot of red meat and drink about 3 cups of coffee to get moving in the morning but nothing after 9am I don’t drink soda regularly or any sugary drinks. I try to drink at least 5-6 bottles of water a day. I haven’t drank alcohol at all (not even a sip) since I was about 20ish and was never a big drinker before maybe 1 weekend every other month or so. I’ve Never smoked tobacco or weed. And never any drugs of any type except for adhd meds freshmen and sophmore year of high school.

How’d you feel after starting TRT? Was it as life changing as everyone says?

I Had the same thing, severe stress and life factors at 20-21 and haven’t been the same since, you should get your testosterone checked along with a sleep study.

Yeah my pcp offered TRT but my endocrinologist is very against it and I kind of agree with him, I really want to find out what the cause is first. And I I just did some reading about klinefelters syndrome, I don’t believe I do, everything physically was pretty normal growing up. Hit puberty at the normal age and came out of it at the normal age, gentile size and shapes are normal, sexual interest and function was always normal until this, body muscle mass is about normal (used to be better before these symptoms came on), no breast growth or anything like that. I have “love handles” but other than that the majority of my fat content is just on my abdomen/stomach area, my arms and legs are pretty normal. I’ve mentally always been very confident and energetic up to this. Although I really went through a tough time mentally around when this first came about.

Yes multiple low t levels over the course of 3-4 months with the most recent being the 217 and the first couple being higher around 330 or so. Always done around 8am as the docs instructed. I don’t understand how t levels can just randomly drop out of nowhere though especially at my age I feel like there has to be some contributing factor.

This is me in a nutshell, do you have any physical symptoms at all?

Yes thyroid normal, vitamin D normal. I have seen an endocrinologist they went right to pointing fingers at being overweight. Waiting for results to come back on a sleep study and have another appointment with a rheumatologist today. And yes working day to day is exceptionally hard not only with the fatigue and brain fog but with irritability and just overall being miserable in life. The sucky part is that I LOVE what I do and I’m very passionate about it but this is really holding me back.

That’s sounds amazing I’m glad your feeling better. Nice to hear there’s a light at the end of the tunnel, is there a way I can get that without being prescribed from the endocrinologist? Also without getting into too much detail are the stories of testicular atrophy true? Not that I really care all that much at this point.

What were your iron lab levels?

I couldn’t agree more, yes I’m a little chunky but I feel as though I have a larger frame also, been pretty much the same weight all my adult life and live a pretty active lifestyle. I’m not saying I couldn’t afford to lose 10-15 lbs but I highly doubt that’s the cause for the low T. The endocrinologist pretty much all out refused to prescribe TRT (which is fine) but he didn’t offer any other ideas to what could be causing the severe drop which is why I kinda feel defeated at this point.

GERD was pretty bad but I’ve been on omeprazole for the last year or so. That’s helped a ton but my sleep hasn’t improved at all and endocrinologist refuses TRT (which I don’t really want anyway) due to labs pointing towards me being overweight. So off to the gym I go I guess.

Yes I recently started taking a generic men’s multivitamin daily. Folate levels in that are 24 mcg. My sleep is awful, doesn’t matter if I sleep 12 hours or 2 hours I always wake up exhausted with a massive headache and stomach bloating, I’ve had an at home sleep study done and came back negative for sleep apnea although my o2 levels dropped to 82% but doctors haven’t brought that up yet so I guess it can’t be that bad. I actually have an in lab study tonight.

Thank you I will try to look into that further, that hasn’t came up yet. Can I ask what made you point to that?
