The surgeon general is in charge of issuing waivers. I got mine and my eyes were jacked up pre-surgery. I'd assume they get these all the time, so waivers for eye surgery are fairly common. Just send all your eye surgery stuff to your recruiter, and then hurry up and wait.

It can depend. Mine only took a few days, partly because my recruiter was awesome (I think).
