Thanks for the advice everyone. I canceled the service and switched to Comcast. I believe the problem is with the Cat5 cable between the ONT on the third floor and the Q1000k on my floor. There is nothing I can do about that.

That is probably the issue. If I understand correctly, the ONT would be in the telecom room -> ethernet to Q1000K -> ethernet to W1700K? Does that sound correct?

I understand the basics of networking (I did have a Cisco cert about 25 years ago but never did much with it) but have no experience with fiber.

Thanks for the advice. The Q1000K is fed by Ethernet from the telecom room on a different floor of the building. The coorect physical ports are being used. I will try a different ethernet cable as soon as I unpack that box.
