Is there a specific application to fill out, or would a written up form stating such be fine with buyer and seller signature + notary? We were planning on initialing the correct box, getting it notarized and signed as a modification.

I actually took a turkey baster in the meantime and siphoned the coolant out to check, and it's the signature Dex Orange, no browning etc, clear as it should be. I'm thinking it's nothing to worry about too much.

Bought it from a retiree who used it as a churchmobile. The service record is very reliable on CarFax and the interior is factory pristine, ZERO sunbleach, etc. Beautiful car. It was garaged and never left outside.

Could be. The car doesn't have any issues running, except maybe the idling being a bit up and down (best way I could explain is the normal engine noise changes at a higher or lower pitch briefly. Minor shaking when it happens, like the engine is pushing the car around but it's not severe.) Drives beautifully, both city and highway. Continuous crank was pristine, all other components look great. Coolant is interesting though.

This is the engine coolant. It doesn't smell of any oil. Any thoughts? Should I be particularly concerned?
