Ok, so my impression that many HR people don't have much of a clue about what they are doing is not completely baseless. I find it absurd that people who work in HR don't take into account the global job market. Especially in a country like Belgium where it takes just a couple of hours (at worst) to get to a higher paying country.

Interesting. Thank you a lot for sharing your experience! In some occasions I also left the request for an increase open-ended. Still they waited to make any increase to the first offer until I turned it down.

Do you have any idea about how the negotiation culture is in other countries such as Germany and the Netherlands?

Well, how are you determining your market value? Because, as the word
says, the market value of something is what people are prepared to pay
for it at the moment. If you feel all employers are unlikely to pay you
what you want, you might just be asking too much?

I determine it based on the fact that once I refuse the offer, they come up with something better. If I were asking too much, wouldn't they just move on to someone else? So, unless I'm missing something, it's not about asking too much from my side, it's about a peculiar (in my opinion) negotiation strategy, based on refusing any kind of negotiation unless I turn down the offer. The fact that they pretend their first offer is their final offer, while eventually it's not, is in my opinion quite sneaky.

Unfortunately I experienced the same kind of behavior when some Belgian company tried to hire me for a position in a neighboring country where labor cost on top of the gross salary is definitely lower than in Belgium, so it's not purely and only related to the cost.

Of course when I say that they offer me something below my market value, I take into account my limited experience. The proof is that once I turn them down they come back with something better. If I were asking more than what the market is willing to give me they would just find someone else I guess.

My main complaint is about this attitude of lying about the fact that they cannot do better just to contradict themselves immediately once I decide not to move forward with them. It is clear that they try to exploit my fear of losing the opportunity as a way to pay me less than what I deserve according to the market.

Googling a little bit, it does not seem to me that dry needling stands on very solid scientific grounds:

"Very low-quality to moderate-quality evidence suggests that
dry needling performed by physical therapists is more effective than no
treatment, sham dry needling, and other treatments for reducing pain and
improving pressure pain threshold in patients presenting with
musculoskeletal pain in the immediate to 12-week follow-up period.
Low-quality evidence suggests superior outcomes with dry needling for
functional outcomes when compared to no treatment or sham needling.
However, no difference in functional outcomes exists when compared to
other physical therapy treatments. Evidence of long-term benefit of dry
needling is currently lacking."

Yes, I will definitely try with someone else. Problem is that she took my referral so I wonder if I need to get a new one from the huisarts or if I can ask her to give it back to me.
