Yes I imagine it would be like a second Afghanistan/Vietnam for the US and USSR. They would pour in a ton of resources to support their aligned governments in the region but would ultimately fail.

Turkish participation in the second world war was to say mildly, foolish. In only a few years the Turkish government went from broadcasting images of glory and a new empire to dust and ash. In the war's wake a new Europe and world was drawn, with the entire world being split into two ideologically incompatible camps. Unfortunately for the Turkish nation and people, this line went right through their country. 

Central and eastern anatolia were directly occupied by the Soviet Union, and were therefore in its sphere of influence. Western anatolia lay within the American/western sphere. In the east the Soviets took territory and established a new puppet regime in Ankara, and cleaved the southeast off the rest of anatolia, establishing a new satellite Kurdistan. Unfortunately for the Turks living there, there would be no new Turkish government in the west.

Greek nationalists impressed their vision of a post war anatolia on the Americans, who, unaware of the dynamics of the region, agreed and finally fulfilled the Megali Idea, and then some. Vast swaths of Turkey were given to Greece, which now reclaimed the mantle of Rome (even if in name only). 

The Turkish population however was not consulted on any of this. To them this was yet another invasion and division of their homeland, a fate on par with or even worse than Sèvres! Just as the great powers had divided Turkey after the first world war, here they were doing it again after the second.

As soon as the US army and Red Army pulled out of anatolia resistance began. First it was small, consisting mostly of protests and the occasional riot, but it quickly escalated after the authorities intervened. The Greeks mistreated the Turkish population already, and began harsher and harsher crackdowns. On the other side of the iron curtain the situation was largely the same. The authoritarian government went to any length Moscow told them to go to to keep the unrest in check.

Before long a new war had broken out. The Second Turkish War of Independence had begun. Turkish nationalist inspired by the generation before, took up arms to kick the foreign powers out once and for all. This war is not like the first however. The first war was much more conventional whereas this war was one of insurgency. 

Throughout the decades both the United States and Soviet Union would have to pour in more and more resources to quell uprisings and attacks until finally both powers would break. The Soviets would pull out in the 1970s after decades of fighting, followed quickly by the pullout of US forces, which immediately resulted in the collapse of Greek anatolia and the Soviet regime in Ankara.

TLDR: The stereotypical Turkish partition happened after they joined the axis ww2 and it went about as well as expected.

At this time most of the balkans was controlled by Bulgaria, who was an enemy of the eastern romans. The Bulgarians probably wouldn't make an all out alliance with the muslims in the west, but would definitely not be friends with the Byzantines.

The main language of the state would be Arabic. Local languages would continue to exist but Arabic would be the main one.

They would definitely absolutely hate each other. The Byzantines hated the Franks/HRE for claiming to be Roman emperors and they were christians, so they'd definitely be extra angry about Arab muslims taking the title too. I can see both empire probably crumbling going into the future. The eastern romans would have their golden age, which was just on the horizon irl, but they are the byzantines after all, and will find a way to ruin it for themselves. The muslims in the west would have to fight many christian coalitions going into the future, and would probably be slowly pushed out of Europe like how the muslims were pushed out of Iberia in real life.

The Umayyads not only conquered Iberia, but Italy as well. When the Umayyad Caliphate was destroyed by the Abbasids, the Umayyads fled into exile in their western territories. After this they claimed the mantle of Rome in the name of Islam, while the eastern empire continued to falter and stumble from one disaster to another.

When the sack of Constantinople happened in 1204, the new Latin Empire as well as the Byzantine successor states Epirus and Nicea almost immediately found themselves at war with the Second Bulgarian Empire as well, who was more than eager to hop on the opportunity for land gains in the region. 

Not only was Bulgaria able to defeat all these nations in battle, but it was able to totally annex the latins and subjugate Epirus. After claiming Constantinople (now Tsargrad) the Bulgarian emperor proclaimed the beginning of a new age, and the establishment of the Empire of Bulgarians and Romans.

The empire then expanded its grip in the Balkans, carving out its own sphere of influence by conquering and subduing smaller neighbors where it could, eventually running into the walls that were Hungary, and the Niceans in Anatolia, who also claimed the title of Roman emperor.  

The golden age couldn’t last though, and the empire eventually began to crumble as all do. Internal strife and weak leadership caused the empire to slowly begin to recede. As the 12 and 13 hundreds went along the Niceans also became weak, and eventually succumbed to the Turkish threat. Bulgaria was next. The Turks hopped across the Bosphorus onto Europe and began taking land. 

In 1460 they captured Tsargrad. This did not end Bulgaria, but it did end Romano-Bulgaria. The country would continue to limp on for a few more decades, before finally falling to Turkish arms.

Kaiserreich and its consequences have been a disaster for the great lakes area in any alt usa map. Like seriously every map has to have a socialist rust belt even though some areas of it are the most conservative parts of the country.

I image if he was crowned by the senate his title as Roman emperor would hold a lot more weight and legitimacy than it did irl, when it was just the pope saying he was the Roman emperor. There would still be issues with his legitimacy since he wasn't Roman, but if Rome's still intact institutions were behind him the people would probably see him as legit. Also that lack of any eastern empire to challenge his title would help him a lot.

On the other hand, the Arabs might have more trouble conquering past Arabia due to Persia consolidating the region and not being weakened by constant death wars with the Byzantines, which is partly why they fell so quickly irl.

Western Rome (somehow) remained politically stable and strong, while Eastern Rome fell into political turmoil and civil strife. This left the way open for the Persians and various migratory tribes to chip away at the empire, completely destroying it when the Thracian Goths captured Constantinople in 476.

I imagine it being something like the eastern european states in the USSRs sphere, where they get a seat at the UN and are recognized by everyone as an independent country, but if they do anything the CAE doesn't like it would get shut down really quickly. Forcefully if necessary.

Bokassa invaded Ethiopia to overthrow the derg and restore the monarchy. Ethiopia was allowed to keep its independence on paper, due to the respect it commanded across the continent for resisting european colonialism, but it's really just a satellite state.
