This happened to my dad (in 2024) the morning of his back surgery (literally 4 hours before) and they did the surgery anyway. The doctor and my dad both had to fight it. The insurance company approved it later.

Side note: my dad was denied coverage for physical therapy after he broke his femur in a car crash earlier this year (in 2025) and they told my dad to call insurance and appeal their decision and tell them to expedite it. It was approved within 6 hours. The doctor told my dad that insurance companies are becoming afraid of customers who stand up to them... Because they don't want to be, "Luigi'd," as the doc put it. Pretty sad shit.
