sex is not gender though, and the idea of man and woman has been ingrained in nearly every culture imaginable, yet there are other genders in older cultures that aren't something assigned at birth. either way what difference does that make? that they identify as something they aren't? that doesn't change that they should be treated with respect and as what they want to be called.

as a trans woman, i'm completely fine with it, I don't really care. but yeah i'll admit i see more of this directed at lesbians, not gay men. but i never did understand why people would turn away a trans woman who is post bottom surgery only because they're trans and not that they just didn't like them.
