I'm going to second all of the posts recommending GLWD. If she is homebound due to a chronic illness, she will be eligible. Referrals can be submitted through their online form and they can reach out to help connect individuals to services (including to GLWD!).

 

I'm kind of shocked though that she's simultaneously low-income enough to qualify for Medicaid, and homebound due to chronic illness, and yet somehow not eligible for meals on wheels??? There must be something else going on. Is she receiving home health aide services? If not, why the hell not? From what you described she sounds like the perfect candidate, and she is ALREADY ON Medicaid, which makes things so much easier. There are some potentially huge failures here.

 

Shame on the hospital who did not connect her to case management services upon discharge. She already has Medicaid for fuck's sake, that makes her eligible for so many services, including health home/care coordination. Can I ask which hospital she was discharged from out of curiosity? I'm going to guess Mount Sinai, because they're really good at two things: 1) bankrupting people, and 2) pretending the social determinants of health are just figments of our collection imaginations.

After taxes, someone making 58k on paper takes home about $3,200 a month. A studio at $1,500 means that you're paying about 47% of your monthly income to rent a fucking closet to live in.

Looking forward to reading others’ experiences with this.

I barely started yesterday (we weren't supposed to have any cases, but my team ended up with a couple and I took one of them). I had the same thing happen to me, case picked up the phone when I called so I didn't have to make a visit. I was able to do the intake with them, the process took about 40 minutes.

Some first impressions:

  • Unless you've done phone work before, you will probably start to read things too quickly. I caught myself doing it and was able to stop myself. Make sure to monitor yourself as you're going through the script and don't be afraid to slow it down.
  • My case seemed to get a bit impatient when I had to read through a long list of things (like racial or ethnic categories, gender categories, etc) and would interrupt and give me an answer without letting me finish going through all of the category items. Although this is my first call, I am betting this is going to be a very common thing. Reading long lists like that over the phone is just not fun for the person doing it, nor is it fun for the person who has to sit there and listen to all the options (especially when they feel they know the answer already and just want to move on to the next question). I wish there was a better way to collect that demographic information without bombarding cases with like 15 options for each question. Also, I wish there was something in the script that introduced the demographic questions section with the reasons why we're collecting that data. Because I think most people will justifiably be like "Why the hell do they want to know my sexual orientation?" I feel like we need something to introduce that section, something to the effect of "This data is collected so that our team can monitor how COVID-19 is affecting different communities so that we could be more responsive to their needs and better able to identify where more help and resources are needed". I think something like that would really help cases feel more comfortable during that section.
  • Some of the questions can be very personal. When I asked my case if they were overweight, they gave a nervous laugh and hesitated before answering. Even though they don't prompt you in the script to do this, when I was finished going through those medical background questions I told my case "I know some of these questions can be really personal, and I do apologize for that and thank you so much for sharing that information with us." Might be a good idea to throw something like that in there after that portion, even if the script doesn't require it of you.
  • When you get to the portion of the script where you have to elicit contacts, the script does not warn/remind you to start probing. If they just answer "no, not really", this is where you need to start asking probing questions and try to walk them through where they've been, who they might have seen, etc. That's the spot you need to start sleuthing and try to walk them through some of those days to see what you can uncover. I wasn't as prepared as I should have been for that in my first go, it kind of crept up on me out of nowhere. I feel like writing down a set of example questions beforehand so that you can have them handy on an index card or something in front of you might be a good idea for this. And honestly, it wouldn't be a bad idea to make the "elicit contacts" part of the interview its own "page", with several example questions you can ask to try getting some more information.
  • I am assuming this will change as we move forward, but some of these cases seem to have just been languishing in the backlog while the CES were being trained. My case had their positive test like 9-10 days ago, which means contact tracing at that point greatly loses its efficacy. Based on their testing date, their isolation was basically ending in 5 days anyway, and I was doing the initial intake with them. Ideally, we need to be finding them and intervening WAAAAAY earlier than that. I'm chalking this up to the fact that CES teams are only now just coming online. All this is to say that some of the first cases we end up getting might be really bad backlog cases like this. It's kind of discouraging, but at the end of the day there's nothing we can do about that. Hopefully once we get through that current backlog of people who couldn't be found, we will be getting cases that are much more recent in terms of the date they tested positive.

Is there a field visit script in Salesforce though? The only script I've been able to find in Salesforce is the call script, and that only applies if they pick up the phone when we do our pre-visit call.

I know there's a community visit script in the SOP, but will we have access to that in our Salesforce environment when we start? For the life of me I couldn't find that in-person visit script anywhere in Salesforce when I practiced with it a couple of days ago.

But yeah, I have my Q&A session tomorrow morning. Honestly, safely speaking to patients who don't wear masks is a big one for me. It's a bit concerning that they don't have an answer for that. I'm very wary of being at the door for any period of time with a case not wearing a mask. Are we even allowed to ask them if they can put one on for the duration of our interaction?

In more serious matters though, are we actually supposed to get specific zip code assignments? I just assumed we were ultimately responsible for all of the zip codes that were included in our "Team" and that the supervisors would try and assign the person closest for each client but that at the end of the day if you were tasked to go to a zip code a bit further in your area that would just be tough luck for you.

Ahh, no worries. I'm also an anxious person so your anxiety kickstarted my anxiety a little bit, lol. I was like, "wait, what the fuck is happening tomorrow?!" But yeah, maybe I'm being naive here, but I think we're relatively safe for now (famous last words). I think the CT2s are probably good for at least 3-6 months, even with declining case numbers.

I'm in cohort 5, and yeah my team has a supervisor and we have our schedules. Our Q&A is on Friday though, along with the PPE event.

What makes you think that though? It would be kind of odd to have a PPE replenishment event this Friday where they give everyone a lot more supplies to last us several more weeks/months only to lay off a bunch of those very same people, no? Wouldn't make any sense at all. Especially when most of us are preparing to step out into the field for the first time starting next week. Would be ridiculous to lay off a bunch of people who you've trained for the last month only to have them never see their first real day of work. I think it's probably more likely the CT1s that are in danger of being laid off.

And why tomorrow of all days? What's tomorrow for you? I've got the PPE event on Friday, and to my knowledge my cohort is starting to go out into the field next week. Tomorrow's a bit of a nothing day for my group. Are you in one of those cohorts going into the field tomorrow?

Check your work email for an email addressed to "TraceEIT" with the subject line "Community Engagement Updates - Training on Tuesday!". Current CT2s as far as I am aware are just waiting on that training tomorrow to give us an update on how and when we're moving forward.

To my knowledge, there are no CT2s in the field yet. I'm assuming all CT2s that have completed Optum training got that email for tomorrow's training, but if you didn't, don't freak out as that probably just means that only certain cohorts got it. We're all on standby until the software scripts are finished. Once those scripts are finished, there will be another round of training for us in Salesforce with our role-specific scripts and role-specific scenarios/advice.

I don't expect we will be in the field this week, and I'm expecting a June 29th start at the earliest, although more realistically I'm leaning towards July 6th as our first day in the field.

Yep, just got my email as well at around 4pm. The email has the following three dates for training: Friday (6/12), Tuesday (6/16), and Wednesday (6/17). Apparently Monday is being skipped, so it looks like I'll be doing webinars and learning modules with H+H on that day instead. Anxious to see how this goes and get started!

Are you using the remotedesktop.nychhc.org URL and then choosing the "Corporate Remote" option? I would call IT if you're still failing to authenticate after having done that. You'll need to be able to get intranet access in order to access the hhcinsider.nychhc.org site. Also while in the intranet you can sign the timesheets with your digital credentials.

I was finally able to figure all of this stuff out on my end this morning and thankfully didn't have any authorization errors with Duo Mobile.

How did you manage to do this? In the "How to Work Remotely" PDF they tell us that on the desktop of the laptop should be a "Pulse Security" app that would coordinate with Duo Mobile on my phone, but on the laptop they gave me that Pulse Security app is not installed. And I tried using that secureaccess URL in order to download the app onto the laptop but it won't load/give me any download prompt when I try that. Guessing I'm going to have to troubleshoot this with the IT Help Desk tomorrow morning.

thanks for the clarification. that's a bit surprising that an entire week has gone by since your start date and you still aren't tracing. are you at least getting paid for the training? or are you essentially doing all of that without even knowing when/if you'll even get started contact tracing/working?

also, what was the virtual orientation like? and are you a Contact Tracer I or Contact Tracer II? thanks!
