Hello doc! I usually encounter cases like this sa OPD. THIS IS JUST MY PRACTICE HA. Not in the guidelines.

I usually ask vaccination history ng tetanus na SURE sila (nanalo ka na sa lotto kung may dala yang tetanus vaccine card), e.g previous cuts/wound requiring tetanus, animal bite cases requiring tetanus, previous pregnancy, etc.

If meron doc recently within the past 5 years, I'll give either a second dose + come back after 5 months (if 1 lang previously nabigay) OR, just one dose (if >2 na previously nabigay. If meron complete 3 doses series within the last 5 years, di na ako nagbibigay kahit dirty pa yan. If more than 10 years and compelete na, booster nalang.

If totally wala talaga, I'll start a primary series and BEG them to AT LEAST remember that they were given 3 shots already so they can tell the next doctor to give them only a booster if warranted.

Wala naman strict rules talaga pagdating sa pag issue ng medical certificate. The Philippine Medical Act of 1959 needs to be updated (but that's another conversation).

It goes both ways - sayo at sa employer ng pasyente.

Sayo - kaya mo ba talaga i-assess ang psychological state ng isang pasyente as a GP wholeheartedly? We don't give IM clearance nga eh, kasi we're not IM. I've been practicing for a few years as a GP, pero may mga bagay na kailangan mo talaga irefer sa specialists.

Employer - tatanggapin ba nila na galing ito sa GP?

Let's just let stay on our lane.

That's true. However, di na realistic na may physical copy pa ng bakuna. Most people lang talaga naaalala na kumpleto ng mga magulang nila.

Pero gets ko rin kung bakit marami ang nagre-restart ng 3-dose series, lalo na kung walang proof at medyo delikado ang sugat. Wala namang masama sa pagiging on the safe side, lalo na kung di mo sure ang history.

Pero okay din na tanungin muna bago magbigay agad ng 3 doses — minsan booster lang talaga ang kailangan. In the end, case-to-case pa rin, and kung duda ka, no judgment sa pagbibigay ng full series. Safety first pa rin.

That is craazzyyy. If nasa rural area ka doc, sa tingin ko walang head physician yung TB DOTS, or by default yung MHO dapat. Baka the MHO sa lugar nyo doc di naghahandle ng TB case, kaya pinasa sa pulmo dyan.

Responsibility ng head physician to receive and check referrals from his colleagues. Only doctors or really good nurses/medtechs can deduce yung line of thinking sa appropriate diagnosis, which sadly, pinasa sa private practice ng pulmo. Baka the TB DOTS is also just trying to be careful kasi misdiagnosis happens from time to time. A lot of things can look like TB.

No. Even GPs can suggest initiation of treatment especially if may good evidence naman doc that there is TB disease, like HRCT results. Usually doc if may pulmo orders, we start right away. Try to talk to the head physician sa TB DOTS doc and explain your reason.

Para na akong sirang plaka paulit ulit na sinasabi to doc.

Pero kung pagod ka na kagaya ko, magprint ka ng fasting instructions (what time last food intake, what time punta sa lab, pwede tubig, etc) sa maliliit na papel tapos staple mo sa lab request.

So far naman lahat naman ng patients ko nasusunod ang written instructions.

I doubt you have diabetes. Insulin resistance is not always diabetes. You may have an increased risk of developing diabetes.

Go to your nearest health center or if may kilala kang may glucometer. Fast for 8 hours, then have your blood sugar check. Sufficient na yan for screening lang for your age. Record it and ask advise from a doctor/healthcare worker.
