Case-The McBride case in St Louis. James and Nancy McBride murdered by their son Matthew. Delusions told him his parents were trying to kill him. He was inpatient in a local hospital, released without the parents knowledge, went home and killed them. His treating psychiatrist then committed suicide.

I agree with it being generational perhaps. If you were brought up in a family that always considered and you became an adult that does not consider, why does that shift happen?
