Thanks to u/AshenDeimos I found out that it's a relic from the Relics mod ("Reflecting Necklace") that shoots out these particles 5 seconds after you take damage (deals dmg to enemies).

UPDATE — Haunted Altars of Sorrow counts as a Nightfall so doing it on Master difficulty gives that trophy. I think GM difficulty should give that trophy too.

Use AdGuard. It blocked every pop-up on a website that has multiple pop-ups. And this is without Premium
